Credential issues with AppInsight for SQL?

I am trying to get AppInsight for SQL to work with one of my SQL 2008 R2 servers and am seeing some things that I don't understand. I apparently had no issues creating the application, but now I am seeing GRAY in some places.

In the first screenshot you can see that the MSSQLSERVER instance has a gray icon on it:

In the second image, you can see that the master db is UP, but the App is Unknown:

And in the third image, both the SolarWindsOrion database and the App are unknown:

Can anyone help me understand what I am seeing here?
